Cancellation Policy

If you require to reschedule or cancel an upcoming appointment you can do so by emailing info@alignindependence.com.au or calling the office on 0406 943 133 directly.
For cancellation fees and cancellation period please refer to the below table. Please note that you will be charged the full fee if you do not attend your scheduled appointment.

Payments

How and when do I make my payments?• All appointments must be paid for at the time of service or prior to the day of appointment.• Payment can be made via cash or contactless payment.• Prepayment via direct debit can be arranged by prior discussion. Payment must be made 2 working days prior to appointment to ensure transfer can occur. • When payments are being made via a third party, such as NDIS, discussion around invoicing is required at time of booking. • Contactless payment - Option 1 ~ Via EFTPOS at the time of appointment. - Option 2 ~ Payment in advance via direct bank transfer. For this option Align Independence requires proof of payment transfer at least 2 business days prior to the scheduled appointment. This can be done by sending a screen shot of the transfer to accounts@alignindependence.com.au. Please contact the accounts office for banking details.

Reports

Invoices/payments for any and all reports or other requested documentation must be made and settled prior to this information being released.

Credits/Refunds

Should you incur a credit, this will be held on file and allocated towards future appointments. If this is not suitable, a refund can be made via direct transfer to a nominated account of your choosing. No cash refunds can be offered.

Feedback

Any queries or feedback please contact the director or email info@alignindependence.com.au. Alternatively, please submit feedback using our online form via this link. Hard copies are also available. All feedback and comments on how to improve service to you now or in the future is highly valued and welcome.

Rebates

No healthcare or Medicare rebates are available at this time. All fees are to be funded privately or via your third party (eg. NDIS, Workcover, TAC).

Confidentiality

Information is routinely gathered as part of the assessment and treatment process, this is seen only by the treating team. The information is securely retained in either electronic form, or hard copy documents are stored in a locked filing cabinet. Under certain circumstances staff at Align Independcence have a legal obligation for manotory reporting of certain information. This is done to protect yourself and others, if you have concerns in relation to this please discuss with your practintioner.
